New SLP Treatment Codes Replacing 92507 in 2027: What Speech Therapists Should Do Now
For decades, most individual speech therapy sessions have been reported with one code: CPT 92507. That is about to change.
According to ASHA, 92507 will be replaced by a new family of 10 speech-language pathology treatment codes beginning January 1, 2027. This guide explains what is changing, how the time-based structure works, and what to do with your documentation and billing systems before the new year.
What Is Changing
Code 92507 describes individual treatment of speech, language, voice, communication and auditory processing disorders.
Under the new structure, it is deleted and replaced by codes that describe treatment by clinical service. Each service has a base code for the first 30 minutes of direct, one-on-one contact and an add-on code for each additional 15 minutes.
Group treatment stays with code 92508, which remains untimed. ASHA notes that 92507 stays in effect through December 31, 2026, and clinicians should not report the new codes before they take effect.
The Five Clinical Categories
The new individual treatment codes cover five categories:
- Fluency disorders
- Speech sound production disorders
- Language comprehension and expression disorders
- Combined speech sound production and language disorders
- Voice, upper airway dysfunction and resonance disorders
When a session addresses both speech sound production and language, ASHA says to use the combined codes rather than the separate ones.
How Time-Based Reporting Works
The new codes depend on direct, one-on-one treatment time. Under the general CPT midpoint rule described by ASHA, a base code can typically be reported after 16 minutes, and each add-on unit after at least 8 more minutes beyond the full 30-minute base period, 15-minute increments after that.
For example, 55 minutes of language treatment would typically be reported as one base code plus two add-on units, while 48 minutes would be one base code plus one add-on unit.
Time spent on untimed services or on a different timed service cannot be counted toward the same code. Payer rules can differ, so always verify requirements with each payer.

Important Caveats Before You Plan
- ASHA’s page showed placeholder code numbers until the official 2027 CPT code set is published, so check the official numbers before you update any system.
- Medicare values are proposed in the 2027 Medicare Physician Fee Schedule rule, and final values come with the final rule.
- Medicaid and commercial payers may adopt the codes on different timelines, and some may keep accepting 92507 for a while.
What It Means for Documentation
Because code selection now depends on the clinical service and the direct treatment time, every note needs to show both clearly.
Your notes should record which disorder category was treated, the minutes of one-on-one treatment for each service, and the goals addressed. Review your SOAP notes templates for speech therapy and add fields for service category and total direct treatment time.
Good speech therapy documentation software can prompt for these details at the point of care, so nothing is missed at billing time. If your current system is slow or rigid, read about EMR for speech therapy problems to watch for.
What It Means for Billing
Therapy billing software will need to handle base codes and add-on units, and to apply payer-specific rules about time. Ask your vendor whether and when its billing tools will support the new codes, how time is captured, and how claims are checked before submission.
ReadySetConnect’s features include scheduling, notes and billing tools, and any platform you use should confirm its readiness before January 1, 2027.
Preparation Checklist
- Review the new code descriptors and CPT instructions once the official code set is available.
- Train clinical and billing staff on code selection and time reporting.
- Update your documentation templates to record service category and direct time.
- Ask your software vendor about its plans for the new codes.
- Contact each payer about adoption dates, coverage, authorization and payment.
- Watch for ASHA updates and the final Medicare fee schedule.
